Home Care Services Offered



We bring a compassionate and committed approach to home care with an elevated level of experience and professionalism.


We know that every client’s health care needs are unique and we are committed to creating a positive experience by developing a Personal Plan of Care with each client.



Personalized services include (but are not limited to):


 

 

Free In-Home Assessment



We offer and require a Free In-Home Assessment for every potential client. This allows us to meet face to face and assess what the best plan of care will be.




A Free In-Home Assessment includes the following:



  • A scheduled visit to the client’s home at their convenience
  • A general assessment of the client’s physical and mental capabilities
  • A safety assessment of the client's home



Personal Plan of Care (PPC)



A Personal Plan of Care (PPC) serves as the daily guide for the client’s caregiver so that they know exactly what the needs are and what level of care is required.


A Personal Plan of Care (PPC) includes the following:


  • A personalized plan tailored to each unique client and their health care needs.
  • All PPC’s are put together by a licensed nurse.
  • A PPC can include the following (but is not limited to): personal hygiene, incontinence care, meal planning and preparation, assistance with errands, socializing support and more.



Bathing, Grooming and Dressing



Bathing, Grooming and Dressing is the most intimate service offered. Our approach to this level of care is to manage the process respectfully and patiently. When assessing the needs for this level of care, we always take the following into consideration:



  • Does the client prefer a male or female caregiver?
  • At what level do the client’s physical limitations restrict them from caring for their own hygiene?
  • How tiring is this process for the client so we can determine the best approach and timing?



Nutrition Guidance, Meal Preparation and Hydration Level Management



We understand that many clients have special dietary needs. Our caregivers are able to prepare home-cooked meals in line with the nutrition guidelines set by the client’s doctor or nutritionist. All meals are thoughtfully planned with the client’s likes and dislikes prioritized.



Clients who are capable are strongly encouraged to assist in the kitchen as a way to ensure a sense of independence and productivity.



Managing hydration levels can be a critical component of nutrition guidance. Some clients may have a health condition, like congestive heart failure, that limits the amount of fluids allowed throughout the day.  Hydration management also includes implementing fluids within time elements in order to maximize hydration without impacting sleep patterns.




Range of Motion and Strengthening Exercises as Allowed by Primary Physician



We can offer assistance with range of motion and strengthening exercises that are authorized by the client's primary physician. This is often needed for clients who receive physical or occupational therapy so that they can continue their rehabilitation at home.



 

Medication Reminders and Vital Signs Monitored



Our caregivers offer medication reminders to ensure that all necessary medication is taken as needed.



Vital signs are monitored throughout the day as required by the client’s Personal Plan of Care (PPC). These can include:


  • Blood Pressure
  • Respirations
  • Temperature
  • Pulse



 

Light Housekeeping, Laundry and Linen Changes



A clean and tidy (well-kept) home creates a stress-free, safe and healthy environment for our clients.


Light Housekeeping and Laundry Services Include:



  • Dusting
  • Vacuuming/Mopping
  • Washing, Drying and Folding Laundry
  • Bed, Bathroom and Kitchen Linen Changes



Transportation



Transportation assistance is offered for all ambulatory clients. Trips to the store, doctor appointments, church, etc. are available. The requirements and limits to this particular service are discussed during your in-home assessment.  We make arrangements for clients that are restricted to wheelchair access only with Orange County Transportation Authority Access Service.




Companionship and Security Assurances



Socialization is essential for the health and well-being of everyone. Our caregivers can provide the necessary companionship to clients who are seeking more person-to-person interaction.



Companionship services can include (but are not limited to):



  • Taking walks with mental stimulation
  • Playing card/board games
  • Going out to eat or other social engagements
  • Assisting with pet care
  • Assisting with gardening
  • Conversations regarding current and past events


Caregivers are attentive to the client at all times which gives the client’s loved ones peace of mind in knowing that they are never alone and safety precautions are in place.




Respite Care for Family or Care Provider



Caring for another person is the most selfless act anyone can commit, but it can also be the most taxing on that caregiver’s health and peace of mind. Everyone needs a break from time to time and our Respite Care service offers just that. One of our caregivers will fill-in while the primary caregiver gets some much needed time to themselves.




Hospice Assistance and Support



Hospice care occurs during a highly stressful time for clients and their loved ones. It is our goal to ensure that the client is comfortable and that loved ones are allowed the time they desire with the client. Our caregivers are on-hand to assist with hygiene, nutrition and overall comfort.




Assistance with Insurance Billing



We understand that insurance billing can be intimidating therefore we do offer assistance with receiving compensation from our accepted forms of insurance.

  • US Veterans Aid and Attendance Benefit
